What is a PCMH?

It's a new concept in primary health care where we prioritize coordinated, patient-focused care across all settings. Our physicians and staff are dedicated to partnering with you to address your medical and non-medical needs.

Key Features:

• Personal Physician Relationship — Your physician tailors care based on clinical guidelines to improve health.

• Care Team Collaboration — Your care team, directed by your physician, ensures efficient, coordinated care, including collaboration with outside providers and specialists.

• Hospital Care — If hospitalized, you will be attended by a Family Care physician who is familiar with your history. Post-discharge, we review your stay and coordinate follow-up care.

Your Role:

• Active Participation — Be proactive in managing your health, follow care plans, and communicate with your physician.

• Appointment Readiness — Be prepared for appointments, bring a list of medications, and adhere to care plans.

Quality Services:

• Access and Convenience — We offer same-day appointments and utilize eClinicalWorks for electronic reminders, ensuring the best care, quality, and safety.
