Job Description

Family Care Associates is a primary care practice that was established in 1990 and remains an independent physician’s office in Effingham, IL. We provide a full range of quality medical care to the patients and families we serve. We are a certified rural health clinic and are a Level 3 NCQA Patient-Centered Medical Home. Our most recent accomplishment was integrating behavioral health into our practice. Now employing six behavioral health providers including, a Licensed Clinical Social Worker, Licensed Clinical Professional Counselors, and a Certified Play Therapist. NURSE LPN/RN - You will be part of our team engaging with our patients, developing relationships providing a unique blend of skills and knowledge to carry out clinical tasks while delivering outstanding patient service and positive patient engagement. Nurses are instrumental in collaborating with the providers and patients helping our patients feel at ease about their healthcare. Primary duties - Ultimately ensures steady workflow and uninterrupted service in our facility. - Triages patient phone calls. - Performs patient Assessments - Notifies and consults with physicians and supervisor. - Follows established patient care plans. - Demonstrates proficiency in skills applicable to nursing. Qualifications Family Care Associates prides itself on quality medical care.
