School Physicals

School physicals are required for those entering Kindergarten, 6th, and 9th grades.
Physical forms are available on our Forms page if you did not receive one from your school.

Kindergarten:

• Physical form. Parents, please complete your portion prior to the appointmet.
• Urine test. Please collect first morning urine in a clean container (patient name and date of birth must be on the container). Keep refrigerated until the appointment.
• Immunizations. Dtap, Polio, MMR, and Varicella are required if not received at 4 years of age.

Sixth Grade:

• Physical form. Parents, please complete your portion prior to the appointment.
• Urine test. Please collect first morning urine in a clean container (patient name and date of birth must be on the container).  Keep refrigerated until the appointment.
• Immunizations. Tdap and Meningococcal I are required. We also recommend Meningococcal II and HPV vaccines.

Ninth Grade:

• Physical form. Parents, please complete your portion prior to the appointment.
• Urine test. Please collect first morning urine in a clean container (patient name and date of birth must be on the container).  Keep refrigerated until the appointment.
• Immunizations. No shots are required but we recommend Meningococcal I and HPV if not given at 6th grade.

Twelvth Grade:

• Immunizations. Meningoccal I is required if no prior doses have been given. A booster dose is required if the first dose was given at 16 years of age or younger. Meningococcal II and HPV are recommended if no previous doses have been given.
